I understand that I will accept full responsibility for any decisions I make regarding my life, and i will not hold Maria Malec or Aroma-Living by Design, LLC or any agency she represents responsible for any legal action, tort, including invasion of privacy, outrageous conduct, intentional infliction, or emotional distress that may arise

  1. 1 understand that the State of Florida mandates that "all professionals must report or cause a report to be made and cannot keep silent on the grounds of confidentiality or privileged communication,* in the following matters. When a disclosure indicates a client may cause danger to sell. When a disclosure indicates a client may pose a danger to others. In all instances of child, spousal or elderly abuse.

    1. That I desire to take full advantage of Maria Malec/ Aroma-Living by Design, LLC services and understand that the use of essential oils may be used aromatically, topically or internally within a session. I hereby authorize permission to use the oils at my discretion or facilitated by Maria Malec/Aroma-Living by Design, LLC.

    2. I understand that Maria Malec's/Aroma-Living by Design, LLC time is valuable. A 24-hour advanced notification is required to change or cancel an appointment and I am to be present and ready to begin program sessions at the scheduled time. At the discretion of Maria Malec/Aroma-Living by Design, LLC, my failure to comply with these terms is just cause for my termination as her client and that in such case i do, hereby, waive any claim to recourse.

    3. I agree that all fees outlined, below, shall be viewed as payment for services rendered to Aroma-Living by Design, LLC for the continued support of informed Self-Care 6. I acknowledge that prior to entering into any wellness program or disclosure. that Maria R. Malec has fully and to my satisfaction explained the above to me
